Location of incident: please include general area (for instance: Boggs, Konocti, Highland Springs)
Examples of Type of incident: Geographical hazard such as fallen tree, man-made debris such as trash dump or barbed wire, erosion creating hazardous footing, or user conflict (such as another user engaging in activity which made you feel that your safety was threatened)
